Checklist for Success

Much of the information required by the following checklist will provide the basis of the Support Plan

1 Job analysis undertaken (individual task analysis if required)
2 Confirmation of tasks agreed
3 On-the-job training determined, clearly identified roles and responsibilities and timeframes for review
4 Training to any co-workers and supervisors
5 Agreement on provision of feedback to the new employee
6 Agreement on a reasonable timeframe for review
7 Support plan is determined and agreed
8 Workplace adjustments are agreed and review schedule confirmed
9 Co-workers are briefed if required
10 Agree on action to be taken and responsibilities if there are changes to the workplace
